緒 言
離チオクト酸を測定し，ついで10昭チオクト酸投与によ
既に本報告の第１報および第２報に述べた如く，皮膚
る負荷後２時間迄に尿中に排泄された総チオクト酸量を
疾患に際し屡々認められるビタミンＢ群代謝の異常は必

ずしも共存する肝障害のために起因するのみならず，

ＴＣＡサイクルの関与する皮膚組織呼吸系のおよぼす影
投与するとその大部分が負荷後２時間以内に排泄され，

実験方法
チオクト酸定量委員会制定のStreptococcus
faecalis
による微生物定量法は血中および尿中の遊離チオクト酸

を測定するに止まり，代謝型たる結合型，あるいはチ
オクト酸分解産物の測定をなし得ないため，これのみで
しかも投与されたチオクト酸は尿中には大部分か結合型

チオクト酸代謝の全貌を把握することは不充分である．
乃至分解産物として排泄されるので，遊離チオクト酸の
消長のみでチオクト酸代謝異常の判定は極めて困難であ
るか，今日の段階では他に方法がない．

今回は各症例を夫々疾患群別に分類し，各々の尿中遊

即ち静注によるとチオクト酸10昭投与の負荷試験後の

尿中遊離チオクト酸排泄量を測定し，皮膚科領域各疾患

群の平均値と標準偏差から観察すると，負荷前では慢性

常域に復した．
［XII］ その他の疾患
均値が対照に比して有意に低かつたが，貨幣挾湿疹の排
泄平均値は負荷後で有意に上昇していた．
第10∼12表に示す如く，その他の疾患は症例か少数の
ついで主なる疾患群についてチオクト酸尿中排泄を負
ため標木平均は無意味であった．痴は負荷前後を通じて
荷前，負荷後の排泄量を各区間に分割して症例数により
比較的低値をとり，肝斑，口唇炎，結核などは負荷後高
度数分布表を作成し，つぎに述べる様な再衿討を加えた
値を示す例か幾分多かった．

各疾患群の平均値，標準偏差などでは知り得ない特徴
ある分布の傾向を度数分布により更に追究すると，健康

人の尿中チオクト酸排泄は適時尿では平均18m7であっ
たか，もしlOnryり≠１）毎に区切って見ると,

以下の者は僅かに20％であったのに対し，亜急性湿疹患
者では41.3%,慢性湿疹は52.4%,円形脱毛症では50.0
％にも達していた．同様の傾向は負荷後尿においては
TA ＳＫｃｒｅｆｈ乃7nぴｒｉｎｅ
さらに著しく，正常人の平均は27.lrであったか，前者
と同様に107ら7≠1）を単位として区切って見ると，

107以下の低値を示す者吐全被検者のr.5%に過ぎない

に反し，亜急性湿疹では30.6%,円形脱毛症は26.r%と
可成りな差が見られた．その百分率は正常人のそれに比

し有意の差で大いなる隔たりを見せた．その原因は各疾
患群の平均値，標準偏差のみならず，その分布型か根本
する傾向か見られた．これは湿疹例の大部分か正常例に
的に各々異なるためと考えられたので，健康対照と湿疹
比し比較的低値をとっているにも拘らず，他の１群の症
例との度数分布曲線をさらに詳しく追跡することとした
例が正常例と同値あるいはそれ以上に増加することを意l
（:第２図）．
味し，先に述べた如く貨幣状湿疹例の負荷後平均値が正
即ち湿疹例は正常に比し，負荷前，負荷後を通じ何れ
常群のそれに比し有意に上昇していたことと共通する点，
も曲線の頂点か低値に移動し極値の尖鋭度も高いが，逆
があるものの如くである，各種疾患群において，かくの
に曲線の周辺部では却って正常よりも分散か大きく，特
如く既に棄却限界を超えた異常な高値を示す症例を屡々
に負荷後排泄量においては今一つの新たたる極値を形成
見出し得ることは興味ある事実である．このことは負荷?
