この特典は、キャンペーン開催期間中にケーブル会社/Freeviw(UK) 経由でJSTVをご契約頂き、
JSTVカスタマーサービスまでEmail、Fax、郵送でキャンペーン特典希望の旨と、「JSTVを契約したことを
示す証明書類」（契約開始日が明記されたもの）をキャンペーン開催期間中にお送り頂いた方が対象で
す。
ケーブルテレビ会社/Freeviewに6ヶ月分のご視聴料をお支払いされましたら、ケーブル会社/Freeviewが
発行した「JSTV視聴開始月から6ヶ月分のJSTV視聴料支払い証明」（Statement)をJSTVカスタマーサー
ビスまでお送り頂き、受領してから適用となります。
キャッシュバック方法は、クレジットカードとなります。別途JSTVにクレジットカード情報をご登録頂きます。
JSTV、特約店の社員ならびに関係者はキャンペーンの対象となりません。
この特典は他者に譲渡、換金することは出来ません。
再入会の方（ご入会月から、過去6ヶ月以内に退会された方）はこのキャンペーン特典をご利用になれま
せん。

Terms & Conditions
1. This promotional offer is open to new JSTV subscribers through Cable TV companies / Freeview
during the promotional period from 1st October 2016 until 31st December 2016. To apply for this
offer, please contact to JSTV Customer service during the campaign period by Email, Fax, or
Post, with a proof of contract including the start date of the JSTV subscription as issued by the
Cable TV Company / Freeview.
2. After paying the 6 months subscription fee to the Cable TV company / Freeview, please submit the
payment statement for the first 6 months JSTV subscription fee as issued by Cable company /
Freeview to JSTV customer service. Upon receipt of your statement we will apply the offer.
3. We will refund 1 month subscription fee to your credit card. Please note that you need to register
your credit card details with JSTV.
4. Those companies which have business links to JSTV (JSTV agents etc.) and their staff can not
apply for this promotional offer.
5. This offer cannot be transferred to another person or be converted into money.
6. This promotional offer is not open to re-joiners who have cancelled a JSTV subscription within the
past 6 months.
7. You cannot use this offer in conjunction with any other promotional offer.
